Assessment

Being diagnosed with a chronic or terminal health issue or suffering a traumatic injury can be overwhelming for the patient and those who care for them.

NCC’s Case Managers are there to help you understand the needs, connect you to resources, and to make certain that your family is receiving the best care possible.

All of this begins with a comprehensive assessment of the client and their caregivers.

Our Case Managers take their time getting to know the family through relationship development and a thorough needs assessment.

Resource Connection

Navigating the healthcare and support systems can be overly complicated and frustrating.

After assessment, NCC will identify, connect, and guide the client and their caregiver with the resources necessary to improve their quality of life and health outcomes.

NCC focuses on the well-being of both the client and their caregivers. We make sure they are both receiving the medical, emotional, and physical support needed.
