Comprehensive Case Management

Neuro Community Care’s (NCC) case management services are designed to meet the unique needs of each client and support their progress toward tangible life changes. Case Managers work collaboratively with clients, caregivers, and providers to develop goals, access services, and provide advocacy. Functional areas that may be addressed include health and wellness, physical and cognitive functioning, independent living, emotional regulation, recreation, and social and vocational skill development or other Social Deteriminants of Health (SDoH). Case management services can be short-term or long-term in an individual’s home, community, or other living environments.
